Collaborative involvement

Our practice is participating in the National Primary Care Collaborative (NPCC).

The aim of this collaborative is to assist practices to develop sustainable and systemic improvements in the care provided to patients and their communities. The three areas that the NPCC is targeting are improved outcomes for Diabetes, Coronary Heart Disease (CHD) and access. As a result of our participation in the NPCC, the practice has continued the Diabetes Clinic and added another nurse led clinic for patients with CHD.

Dr Sandra, practice manager Donna and Nurse Di have made presentations about our clinics at NPCC learning workshops in Melbourne and at the Gold Coast.

Other Practice Initiatives

In Home Health Assessments – Practice nurse Sue has been out and about in the new practice car performing compressive health Assessments for our patients aged over 75 years.

In 2006 practice nurse Di received Australian Practice Nurse Association award for ‘Best Practice in Chronic Disease Management’

Cardiac Rehabilitation & Prevention Programme – Dr Sandra & Di are involved in this programme which is run by a partnership between Qld Health and The University of the Sunshine Coast. The programme is a part of the discharge planning process for acute coronary syndrome, angio and cardiac surgery patients. Dr Sandra and Di’s role in this programme is educating patients about the role that their GP plays in their rehabilitation process.
