PATIENT RESPONSIBILITIES

Patients have a responsibility to treat all practice staff and medical practitioners with courtesy and respect.

Patients have a responsibility to provide accurate and complete information about current and past illnesses, medications and other matters relating to their health.

Patients have a responsibility to advise the medical practitioner or staff if they have a concern about their treatment.

Patients are responsible for their actions if they refuse treatment or do not follow the medical practitioner’s advice, recommendations and instructions, including failing to attend consultations with specialists after being given a referral and failing to have pathology tests performed.

Patients have a responsibility to attend scheduled appointments or to cancel unwanted appointments at the earliest possible time.

Patients share the responsibility to follow up the results of tests or investigations with their medical practitioner.

Patients have a responsibility to provide a current Medicare card, Pension card, Health Care card, Health Fund card as applicable, when requested.

Patients have a responsibility to pay for the services provided or if applicable to sign a Medicare, DVA voucher, as appropriate, at the conclusion of each consultation.

PATIENT RIGHTS

Patients have the right to be treated with respect, dignity and consideration regardless of age, gender, sexual preference, religion and culture.

Patients have the right to information regarding their medical treatment, and the costs associated with their treatment.

Patients have the right to request a medical practitioner of their choice or to change their medical practitioner.

Patients have the right to request a second opinion.

Patients have the right to schedule a follow-up appointment if the allotted time for their consultation has been exceeded.

Patients have the right to privacy, to be interviewed and examined in surroundings designed to ensure privacy.

Patients have the right to request or refuse the presence of a third party observing or being clinically involved in the consultation.

Patients have the right to confidentiality of medical records and personal information.

Patients have the right to access their medical record according to the Privacy legislation and the practice policy.

Patients have the right to approve or refuse the release of information from their medical records except when the release is required by law.

Patients have the right to provide feedback to the practice.
